Performance Measures
To provide for the personal identification of persons entering corrective services facilities.
All staff and visitors to a corrective services facility and offenders must be clearly identified to maintain the good order and security of the facility.
Note: Colour code refers to the colour of the government logo bordering the photograph and not the background colour against which the bearer was photographed.
QCS ID cards must provide the following information-
QCS will issue the following types of ID cards, colour coded to identify access level of the bearer-
Visitor identification passes may be issued by facilities for use by persons visiting a facility who have not been issued with a QCS identification card, for example, personal visitors to offenders. Visitor passes do not bear a photograph and are issued for a single visit only.
Visitor passes must be displayed as required by s 160(2) of the Corrective Services Act 2006 (CSA) and this procedure.
The general manager of a corrective services facility must establish a record containing each offender's details. Refer CSA s 10
A corrective services officer must provide an offender with an offender identification card. All offenders are required to carry his/her offender identification card. This identification card will identify an offender as he/she moves throughout sections of a facility.

The following details must be placed on the offender's identification card-
The digital offender photograph within the offender management system provides the most current photograph of an offender and must be used for all identification purposes.
There must be no other information added or affixed to the offender's identification card.
Offender identification cards must, if applicable, be issued by the facility reception store staff and the issue recorded in the offender's property data on the offender management system.
An offender who significantly changes his/her appearance or loses or damages his/her offender identification card must report the matter to a corrective services officer. A new identification card must be issued and the offender may be required to meet it's costs.
Digital offender photographs (including photographs on identification cards) must be no older than two years.
On discharge or transfer from a corrective services facility, an offender's identification card must be destroyed. The destruction of this card must be recorded and must, at a minimum, provide details including the offender name, date the card was issued and date the card was destroyed.
A security clearance must be obtained for all QCS employees prior to commencement of his/her employment.
All QCS staff must be issued with a QCS ID card as soon as practicable following commencement of employment.
Employees of other Government agencies who are transferred on secondment to QCS must be issued with a QCS ID card for the period of his/her secondment.
Staff on secondment from another QCS facility or unit will not be issued with an update or a second identification card. If required, it is suggested that a letter of secondment is carried.
Queensland Health Offender Health Services Branch employees must be issued with green QCS ID cards. These cards must be worn at all times whilst working within the corrective services facility.
Queensland Health staff engaged to provide services to the Princess Alexandra Hospital Security Unit must produce Queensland Health identification cards to gain entry to the Princess Alexandra Hospital Security Unit.

The general manager of a facility or the Human Resource Services Branch must provide administration of identification systems for any person authorised to visit a facility.
A person wishing to enter a facility for professional, official or other business purposes must provide proof of identity prior to being granted access. They must complete Form 27(a) - Application to Visit - Professional, Official or Other Business Purposes (Part A). Visitors who do not have a current QCS ID card must be issued with a visitor identification pass prior to entry to a facility. Refer Corrective Services Regulation 2006 ss 24-28
A person wishing to enter a facility for professional, official or other business purposes will come under one of a number of categories (refer CSA ss 164-171). These visitors, if attending facilities on a regular basis should be issued a yellow ID card. All other visitors must be processed using the usual visits procedures and must be issued with visitor passes (refer procedures - Personal Visitors to Offenders; Visitors to a Facility (Excluding Personal Visitors)
Signs must be visible at corrective services facilities instructing visitors who hold yellow cards that he/she must reapply for a new card at least one month before the expiry of his/her current card.
The Stakeholder Liaison Officer must notify stakeholders who hold yellow cards that they must reapply for a new card at least one month before the expiry of his/her current card.
When reapplying for a yellow card, professional visitors must complete administrative forms - Identification Cards for Staff and Approved Visitors and Form 27(a) - Application to Visit - Professional, Official or Other Business Purposes (Part A and B); and be re-approved by the Executive Director, Offender Programs and Services for continued service provision (refer procedure - Visitors to a Facility (Excluding Personal Visitors)).

The Human Resource Services Branch must process requests for new and reissue ID cards.
All applicants for QCS ID card must obtain a security clearance prior to the issue of an ID card, using administrative form - Criminal History Disclosure and Consent.
Applications for identification cards must be sent to Human Resource Services Branch for processing. Refer appendix - Instructions for Identification Card Applications
A database must be maintained by the Human Resource Services Branch recording details of cards issued, cards returned and lost/stolen and recovered cards.
The database must be audited regularly to ensure compliance with the procedure. Refer procedure - Risk Management
On issue of an ID card the bearer must be provided with appendix - Use and Security of Queensland Corrective Services Identification Cards by the Human Resource and Services Branch. Staff at the facility must advise the bearer of any security precautions that need to be observed when using the card.
Persons issued with QCS ID cards must be advised that they may be required to be enrolled on an electronic scanning or biometric identification system and that they may be subject to security searches. Refer procedure - Search - Visitors
QCS ID cards and visitor passes must be worn at all times while the bearer is in a corrective services facility. The card may be worn on a chain around the neck or affixed to the bearer's clothing from the waist up in such manner that it can be clearly seen.

If lost or stolen, details of the ID card must be reported as soon as possible-
The Human Resource Services Branch must arrange for the replacement of lost, stolen or damaged ID cards.
Immediately upon cessation or transfer of employment or contract, the person to whom the ID card is issued must return the card to a nominated person or immediate QCS supervisor if a specific person is not nominated. Refer CSA s 345(3)(a)
The general manager of the facility where a person is employed/contracted or the responsible manager where the place of employment/contract is not a corrective services facility must ensure that the person's ID cards are immediately returned following cessation of employment/contract.
Returned ID cards must be forwarded to the Human Resource Services Branch for destruction.
If an ID card is not returned to QCS after it's expiry, the Human Resource Services Branch must make every reasonable attempt to recover the card.
