On 28 October 2009, the Queensland Ombudsman tabled a report entitled Justice on the Inside Report - A review of Queensland Corrective Services' management of breaches of discipline by prisoners.
The report was initiated by the Queensland Ombudsman because of the significant impact that breach decisions can have on prisoners' access to privileges and on their progression through the prison system.

The Queensland Ombudsman made a total of 39 recommendations and Queensland Corrective Services (QCS) have developed an Action Plan in relation to each of the recommendations.
QCS is continually looking at ways of improving its processes and practices. We welcome scrutiny of these, and productive guidance by external agencies.
QCS will be implementing all of the recommendations made by the Ombudsman, and has already substantially completed the implementation of 34 of the Ombudsman's 39 recommendations. All remaining outstanding recommendations will be completed by late 2009, with one final recommendation, requiring an upgrade to the Agency's prisoner computer information system, to be implemented by early 2010.
A copy of the Action Plan has been provided to the Queensland Ombudsman.
